This block is used for quickly logging conversations with legal guardians, since only the essential points need to be noted on the form.

For each white form there is a yellow copy sheet, which can be given to parents as a record of a conversation.

The signature of the participants on the line provided saves later misunderstandings and serves as documentation. This also starts the implementation of the measures discussed and the ensures deadline is not forgotten.
